Alvaston Medical Centre is looking for an Advanced Clinical Practitioner Nurse to join our Practice.
The Advanced Nurse Practitioner is part of a clinical team providing face to face consultations and home visits to patients in a primary care setting. The Advanced Nurse Practitioner is a professionally autonomous practitioner managing undiagnosed problems using advanced nursing skills. The post holder will assist in developing protocols and guidelines within the service and work with clinical colleagues in ensuring the highest standards of care for all patients.
Main duties of the job
The purposeof role is to assist the clinical team in the service and delivery of quality caremanagement for the practice population.
Clinical Responsibilities:To use advanced practice skills within consultation to assess, diagnose, advise, treat and refer patients both in Surgery and on Home visits,To provide a comprehensive assessment,Arranging and interpreting appropriate investigations,Develop, implement and embed health promotion and well-being programmes,Support the clinical team in the development of individual treatment plans for chronic disease patients,Identify, manage and support patients as risk of developing long-term conditions, preventing adverse effects on the patients health,Ensure read codes are used effectively,Prioritise health issues and intervene appropriately,Support the team in dealing with clinical emergencies,Recognise, assess and refer patients presenting with mental health needs,Recognise, assess and refer patients with community health needs, developing strategies for effective care pathways,Liaise with external services / agencies to ensure the patient is supported appropriately (vulnerable patients etc.),Support the clinical team with all safeguarding matters, in accordance with local and national policies,Deliver opportunistic health promotion where appropriate
About us
We have a team of 40+ working atAlvaston and our branch site in Aston-on-Trent. We have a strong clinical teamconsisting of 3 partners GP's, Physician Associates, ANP's, Nurses and HCA's and as adispensing practice we have a team of Clinical Pharmacists on site.
We have an external team whichsupport consisting of a Physio, Midwife, Social Prescribers and CareCo-ordinators and we work closely with all of our third party partners.
Of our 11,600 patient base, weprescribe to 3,000 of these.

As a teaching practice we haveregistrars and students on site throughout the year.

Operateunder appropriate clinical supervision of a named GP supervisor.
Providefirst point of contact care for patients presenting with undifferentiatedand/or undiagnosed problems, utilising history-taking, physical examinations, andclinical decision-making skills to establish a working diagnosis and managementplan in partnership with the patient (and their carers where applicable).
Recogniseand work within own competence and professional code of conduct as regulated bythe NMC andRCN General Practice Advanced Nurse Practitioner Competencies (2018)
Undertakeface-to-face, telephone and online consultations for emergency or routineproblems, as determined by the Practice, including management of patients withlong-term conditions.
Participatein all treatment and preventative healthcare services in the Practice, asdelegated and agreed by supervising GP's.
Performdiagnostic physical examinations and treatment.
Identify,signpost, or refer patients at risk of developing long-term conditions,preventing adverse effects on patients' health.
Organiseonward investigation, treatment and referral where appropriate.
Ensurecontinuity of care, arranging follow-up consultations or reviews as necessary.
Attendregular Practice clinical meetings.
Supportclinical team with all safeguarding matters in accordance with local andnational policies.
Undersupervision of Practice GP's, make professional, autonomous decisions inrelation to presenting problems, whether self-referred or referred from otherhealth care workers within the Practice.
Produce accurate,contemporaneous and complete records of patient consultation, consistent withlegislation, policies and procedures.
Reviewand act upon laboratory results and clinical communications from hospitals andother providers.
Undertakecomplex clinical coding.
Recognise,assess and refer where appropriate, patients presenting with mental healthproblems.
Producecare plans for frequent attendees and undertake initial consultations.
UnderstandPractice and local policies for substance abuse and addictive behaviour,referring patients appropriately.
Overseereferral process and intervene where appropriate.
Contributeto administrative element of clinical reports and initial observations, priorto GP appointment.
Beable to justify choice of medication and support patients in use of theirprescribed medication (within own scope of practice).
Beable to understand impact of co-morbidities, other medications and polypharmacy.
Deliverintegrated patient-centred care through appropriate working with the widermulti-disciplinary team and social care network
Conducthome visits and care home visits as necessary (follow adequate training).
Acton alerts and reminders during consultations ( QOF, local enhanced servicesetc.).
AssistPractice in achieving it's clinical targets and objectives in QOF, local enhancedservices etc.
Apply infection-control measureswithin the practice according to local and national guidelines.
