The post holder should be able to manage their own specialist wound care caseload and work autonomously within Trust policies and professional practice, maintaining accurate and comprehensive up-to-date records.
Responsibilities include:
* Carrying out specialist assessment and treatment of podiatry patients, facilitating safe and timely discharges and transfer of care as appropriate.
* Determining a clinical diagnosis, developing, delivering, implementing, evaluating, and modifying individualized treatment programmes, and providing specialist advice, ensuring appropriate pathways of care and communication via liaison and referral to other agencies as required.
* Taking part in the education and training of podiatrists and qualified and non-qualified healthcare/social services/education staff, including medical staff.
* Supporting the Team Leader in the day-to-day running of the Podiatry Service.
* Advising within the wider MDT on the delivery of the service.
* Developing and delivering effective clinical interventions within the specialty of wound care, contributing to clinical practice in the Podiatry Service in line with the current evidence base.
* Managing autonomously a clinical workload of patients/clients at a specialist level, assessing and treating patients, and maintaining clinical records.
To have specialist assessment and treatment skills in areas including but not limited to:
* Assessment, diagnosis, development, and implementation of individualized care programmes for patients with a wide range of acute or chronic health problems such as diabetes, rheumatoid arthritis, neuropathy, vascular insufficiency, and wound management.
* Receiving and processing complex medical information relating to high-risk patients, facilitating diagnosis and implementation of an appropriate treatment plan, providing advice and education as part of the plan.
* Reviewing treatment plans regularly and when appropriate.
* Discharging patients with a care package of education and/or orthotics with the option to self-refer back to care.
To apply evidence-based knowledge within the specialist clinical area acquired through experience and academic study to a specialist postgraduate level. Responsibilities include:
* Acting as a resource and providing advice for podiatrists, other professionals, and colleagues within the MDT about the assessment, treatment, management, and care of patients/clients with multi-pathological illness and/or complex psychosocial needs.
* Working collaboratively with health and social care community and acute colleagues.
* Ensuring effective wound care pathways and transfer of care/discharges.
To proactively facilitate a seamless transfer of care and continuing therapeutic management of podiatry patients between care settings. Responsibilities include:
* Working collaboratively in an interdisciplinary manner within the MDT, debating the blurring of boundaries across professions, and taking on aspects of blurred boundary working where appropriate.
* Developing specialist programmes of assessment, treatment, and intervention for podiatry patients.
* Managing and prioritizing own clinical workload as appropriate, working efficiently to manage professional time and delegating according to the training and competency levels of other team members to ensure effective service delivery.
* Participating in the supervision process with staff on a daily basis and for students as required.
To ensure a high standard of patient care is provided, including implementing and reviewing clinical standards relevant to the work undertaken.
