We have exciting opportunities for you to join one of our forward thinking and innovative Universal Health Visiting teams in Brent.
We have a range of part and full time Band 6 Health Visiting posts across Brent. You will be an either qualified or currently in training for your Specialist Community Public Health Nurse qualification with a passion for public health nursing seeking to new ways of working with families.
Our skill mix teams use frameworks such as MESCH and MESCH Infant2School to deliver our progressive service and continue to seek and develop innovative ways of working with Brent families to enable to children to have their best start in life.
From April 2023 a Recruitment premia has been introduced to attract and retain qualified Health Visitors within CLCH's Health Visiting Teams.
For registered Band 6 Health Visitors, this post is offering a premium of £2,500 payable upon commencement of employment (T&C's apply)
Main duties of the job
We work in partnership with Brent Family Wellbeing Centre and local Maternity services to deliver Best Start for Life. We believe in co-design and family centred care. You will be joining friendly well established skill mixed teams, working with HealthVisitorwho continually strive to work in partnership with families and early years and maternity services and other voluntary and statutory agencies to provide high quality care to achieve positive outcomes for families in Brent.
You will be supported by Team Leaders and Specialist Health Visitors and be offer a recruitment and retention package, £2,500 payable upon commencement of employment (T&C's apply).

6. To plan, organize and evaluate an innovative health visiting service, and where appropriate, delegate to other members of the health visiting team to ensure delivery of a high quality service
7. To provide a skilled, effective and evidence based health visiting service to clients in a variety of community settings.
8. To work in collaboration with other health care and service providers to ensure seamless delivery of services for clients and communities
9. To collaborate with team colleagues in order to deliver the Quality Strategy objectives.

Qualifications
Essential
10. Registered Nurse / Miwife with current UK NMC registration
11. Registered Health Visitor (SCPHN)
12. Evidence of continuing post-registration education and training
13. Practice assessor/supervisor training or working towards
Experience

14. Health visiting experience
15. Be able to co-ordinate health promotion initiatives in line with local and national health improvement targets
16. Experience of working collaboratively as part of a multidisciplinary team and inter-agency working
17. Experience of organising and managing own workload and that of others ? Experience of assessing, planning and implementing programmes of care and writing care plans
18. Experience in providing practice based learning support to others ? Experience of evidence based care
19. Experience of supervising staff / students

20. Knowledge of recent developments in primary care ? Knowledge of current health and social care agenda relevant to area of work and professional practice
